Clinical Results of Raz Bladder Neck Suspension for Stress Urinary Incontinence

Abstract
Background: Ever since Pereyra described needle suspension of the bladder neck as a form of surgical therapy for the treatment of urinary stress incontinence in women numerous modifications have been presented. Each of these modifications has helped to found healthy anatomical and surgical principles that ensure patient safety, decrease morbidity and improve effectiveness. A late modifications has been described by Raz. But the long-term success rate of Raz approach was not defined. Therefore we evaluated our result with a special interest in the long-term follow-up.

Methods: The 34 women with genuine stress urinary incontinence underwent Raz bladder neck suspension from May 1994 to May 1997. All patients were assessed of their age, preoperative beeded chain cystourethrography, preoperative M-M test and Q-tip test, operative time, complication, and success rate.

Results: Mean age of patients was 49.1(35-72) years. Severity of incontinence were grade ¥° in 2, grade ¥± in 29 and grade ¥² in 3 patients. The success rate were 85.3%(29/34) after 3 months, 80%(24/30) after 6 months, 75%(15/20) after 12months and 60%(6/10) after 24 months. The overall success rate was 73.5% at mean 17.3 months of follow-up.
Complications included prolonged suprapubic pain(23.5%), weak urinary stream (11.8%), vaginitis(2.9%), prolonged retention(2.9%) and prolonged hematuria(2.9%).

Conclusions: The Raz bladder neck suspension for correction of stress urinary incontinence has been successful, but the success rate decreased in this results according to follow up duration. Longer term results within this data will be reported in the future.
